About the Role

South Rural PCN are keen to recruit an experienced general practice nurse to work at the Holbrook and Shotley practice.  The role is ideally full time for 37.5 hrs although part-time may be considered.  The applicant will be expected to work across both sites.

The candidate will work as part of a team providing a range of nursing assessments and provide appropriate care/treatment in conjunction with GPs according to practice policy and protocols.

Duties will include;

  • Lead long-term condition management across the PCN, supporting quality improvement and assurance initiatives.
  • Support public health and screening programmes, including immunisations, vaccinations, and cervical screening.
  • Embed population health management approaches within general practice, targeting health inequalities and improving access.
  • Support, mentors, and supervises new-to-practice nurses and other healthcare professionals within the PCN.
  • Work collaboratively across the PCN to enhance integration with community, secondary, and social care services.

About the Candidate

All applicants should have the following qualifications, registrations, skills and experience:

  • Holds more than one academic Level 6 diploma or postgraduate certification in long-term conditions care and/or public health initiatives.
  • Demonstrable experience in leading long-term conditions management and quality improvement within a PCN.
  •  Registered with the NMC and maintains revalidation in line with NMC requirements.
  • Working as a registered nurse level practice as described in the primary care and general Practice nursing career and Core capabilities framework.

About the Role

South Rural Primary Care Network (PCN) are looking to welcome an experienced paramedic to the team on a full-time basis, working 37.5 hours a week. This can be worked across either 4 or 5 days to suit the applicant. The role will be based at the Holbrook and Shotley Practice and you will be expected to work at both sites. 

To be considered for this role, you will need to have 2 years consolidation of learning period completed as a newly qualified paramedic and a further 3 years experience as a band 6 (or equivalent) paramedic.

Working within the community, as an autonomous, accountable paramedic, you will provide assessment, management and treatment to our patients.

Duties will include;

  • Assess, manage, treat, refer and/or signpost patients/service users who attend surgery with undifferentiated or undiagnosed condition relating to minor illness or minor injury.
  • Use advanced clinical skills to provide education to service users, promoting self-care and empower them to make informed choices about their treatment.
  • Manage and treat patients presenting with minor illness.
  • Triage and treat patients, making any necessary referrals to other members of the primary health care team.
  • Advise patients on general health care and minor ailments.
  • Perform investigatory procedures needed by patients and those requested by the GPs.
  • Communicate at all levels within the team ensuring an effective service is delivered.
  • Maintain accurate, contemporaneous healthcare records appropriate to the consultation.
  • Actively participate in mentoring and supervision, plus practice-derived CPD opportunities.
  • Contribute positively to the effectiveness and efficiency of the team and work colleagues.
  • If a prescriber – prescribe/issue medications as appropriate following policy, patient group directives NICE (national) and local clinical guidelines and local care pathways.

Our PCN provides services to 56,000 patients across rural Suffolk. Our PCN includes the following practices: Bildeston Health Centre, Constable Country Medical Practice (East Bergholt and Capel St Mary), Hadleigh Health Centre (Boxford), The Holbrook and Shotley Surgery and Needham Market Country Practice.
